Once considered the first-choice treatment, hormone therapy has come under criticism in recent years — and for good reason, as there are potential risks associated with the treatment.

If the physical and emotional symptoms of menopause are caused by decreasing levels of hormone production, it would seem sensible and logical to simply replace the missing hormones. For this reason, hormone therapy was previously used to treat all menopause symptoms — regardless of how severe they were.

The benefits

Supporters of "hormone therapy" (HT) believed it offered positive effects — for example on the cardiovascular system and on concentration — beyond simply easing the symptoms of menopause.

More importantly, however, hormone therapy always comes with the possibility of serious risks.

What to consider

Due to the increased risks it poses (e.g., for stroke, thrombosis and sometimes breast cancer), HT is now considered on a case-by-case basis and is only used following an individual benefit-risk analysis. Hormone therapy should only be used for moderate/severe symptoms that are particularly distressing and should be used at the lowest possible dose for the shortest possible time.

According to the current guidelines of the Drug Commission of the German Medical Association, hormone therapy is now only prescribed

  • For very severe symptoms
  • At the lowest possible dose
  • For the shortest possible time

It is good to know that there are well tolerated non-hormonal treatments available for women who do not want to use hormone therapy or cannot use hormone therapy due to an existing health condition. These can help relieve menopause symptoms — and in some cases, even make them disappear.
